Signing Up
You must apply to MSP Teleplan for permission to bill through our service.
Simply phone Michael Young at (604) 473-0046 ext.3 (9:30-4:30 pst) and give him your details. He will look after the signup process.
 
Or download the MSP Teleplan assignment form from the link below.
Print it out, Fill it out and fax 1 copy to MSP and fax a copy to MediClaim at 604- 929-3256
On your faxed copy to MediClaim - write "NEW APPLICANT" on the form and give us an easily reached telephone number and the time you prefer to be contacted.
